Service and Assistance Dog Program Interest Form

Covid-19 continues to impact, industry-wide, the number of available Service Dogs. NEADS is working on increasing the number of dogs in our programs, and, as we make progress on matching our highly trained Service Dogs with clients who have already been accepted, we will open applications.

Once applications do open, the average waiting period for a NEADS Service Dog could be 24-36 months after application, interview, and acceptance into the program.

With this understanding, if you would like to be contacted when applications do reopen, please confirm your interest by submitting this Interest Form. Please note that this Interest Form is not an application. If we do not hear from you by March 15, 2022, we will assume that you are no longer interested in receiving further notices about NEADS Service and Assistance Dog programs.
